Goal

The aim of the project is to promote social inclusion, combat poverty and any form of discrimination by increasing access to affordable, sustainable and high quality health and social services, through the organization of public health programs and screening services for the identification of cardiovascular risk.

Objectives

Raising awareness of the importance of preventive healthcare through information, education, and mobilization campaigns aimed at both the general public and specific target groups.
Promoting the principles of social inclusion, non-discrimination, and equal opportunities in the provision of medical services by integrating these values into training programs, screening methodology, and patient support activities.

Main activities

Developing a unique methodology for identifying patients at cardiovascular risk, which will include assessment tools, implementation strategies, standard forms, and an annual reporting model. This activity involves consultations, working groups, and validation of documents by experts.
Computerization and digitization of the screening process: Development of the National Registry of Patients with Cardiovascular Risk, integrated with national health systems, with secure access for providers and extensive reporting and analysis capabilities.
Training of 900 specialists: Organization of training courses (theoretical and practical, including e-learning), based on an accredited curriculum, for specialists who will carry out screening activities at national level.
Provision of medical screening services for 165,001 people: Effective implementation of the screening process: identification of individuals, referral to their family doctor, consultations, laboratory investigations, lifestyle counseling, referral to a specialist (for approximately 4,000 cases).

Over 4,000 people referred to specialist consultations and actively supported in accessing treatment.
8 regional screening centers established, equipped, and fully operational.
Unique screening methodology, validated at national level, applicable in all regions.
An accredited curriculum for training specialists involved in cardiovascular prevention.
The National Registry of Patients with Cardiovascular Risk, operational and integrated with health information systems.
National information and awareness campaign, carried out in all development regions.
Reports, analyses, and recommendations for public policies based on real data from the field.
A replicable intervention model focused on equity, prevention, and active support, which can be extended to other areas of public health.
